Description:
Provides free screening and enrollment services to individuals for all federal, state and county health coverage programs. The Direct Care program is available for PBC residents who are uninsured, have an income 200% or below the Federal Poverty Level and do not qualify for any Federal, state or County health coverage program. Staff assist patients, address barriers to care such as access to healthy food, transportation and medication.
