Provides practical information, advice and legal tools needed to ensure end-of-life issues will be respected. The Five Wishes document helps express a person wants to be treated if they are seriously ill and unable to speak for themself. It lets family and doctors know:
1. The kind of medical treatment you want or don't want.
2. How comfortable you want to be.
3. How you want people to treat you.
4. What you want your loved ones to know.
5. An advance directive or living will that tells how you want to be treated if you are terminally ill and unable to speak for yourself.
