Description
Facility is licensed under the Specialized Alzheimer's Services Adult Day Care Act. Individualized Plan of Care with mandated updates at least quarterly for each patient.A Registered of Licensed Practical Nurse is on site at least 75% of the hours of the day care operations. Monthly health assessment conducted on each patient monthly. One staff member for every five patients. Direct Care staff receives 8 hours training on ADRD within 6 months of hire. Therapeutic, social, spiritual, physical and cognitive activities are conducted each day. Transportation coordination. Fee may apply based on income and program enrollment criteria.
