Description

Provides general preventative and restorative dental care to low income, adults and children without dental insurance and those who cannot afford treatment as well as Medicaid patients.

Hours

Monday-Thursday By Appointment / 8:30 am-4 pm. Friday By Appointment / 8:30-11 am. Free Extractions, a limited amount of people are seen by appointment only

Required Documents

Must bring to the clinic some type of written document showing financial need of help. Such documents include: official copy of recent tax return, letter of non-filing from the IRS, current Medicaid card, for non-residents of USA, letter from a pastor or priest stating financial need, photo ID. Will not accept food stamp card or letter showing current eligibility for food stamps, pay stubs from employer, social security or disability statement or W-2 forms.

Eligibility

Adults and Children. Complete care available to Medicaid and low income patients. Must be at or below the 200% poverty level as established by the State of Florida Health Department. Call the office to find out the current amount for the size of family.

Fees

Call during office hours for rates. Financial eligibility guidelines based on family size and income. Sliding scale. Will be required to prove financial need in order to receive free services or services at a reduced cost.

Intake Procedure

New patients are screened on Thursday 8:30-11 am and must show a photo ID and proof of income. By appointment only.
