Resident of Palm Beach County, no insurance and 200% of or below poverty level.

No fees charged to qualified.
Intake Procedure
Must bring a photo ID, proof of family income, and proof of address. First, come/first served. Call to confirm hours of operation and schedule an appointment. Press O for the receptionist if they do not want to hear the entire voice message, a voice message is very long.
