Description
Provides medically supervised daytime care for elders who need enhanced socialization, and/or who have mobility issues or mild to moderate dementia. Transportation, snacks and hot lunch provided. Staff includes a Registered Nurse, a Certified Activities Director, Certified Nursing Assistants and experienced recreational aides. Medical management includes administration of meds, glucose and blood pressure monitoring. Can be an alternative to nursing home placement for impaired seniors, while offering respite for the caregiver. A monthly caregiver support group is also available.

Case Management
Description
Case Manager identifies senior's needs and matches needs to services available in the community. Provides periodic contact through home visits and telephone calls to make sure needs are being met. Care managers arrange for Medical Alert devices for elder clients. And, our geriatric care managers assist with the placement of an elder into an assisted living or nursing home facility. A care manager provides counseling, guidance, education, home/friendly visits, long-term care planning, and advocacy services. The care manager is an intermediary for families here and out of state. The rates for a personal care manager are as follows: Initial Assessment : $125./ individual - $200/couple Professional geriatric care management services: $60.00/hour

Description
Provides a hot, noontime meal and socialization for seniors in select neighborhood locations, Monday through Friday. No fees are charged for the lunches, but donations are accepted. Description
Emergency Home Energy Assistance for the Elderly Program (EHEAEP). Provides financial assistance to senior citizens for past due electricity bill. Allows a one time payment during a heating season and a one time payment during a cooling season.

Description
Personal Emergency Response Systems is a Medical Alarm that allows independent senior adults to receive assistance when they require it. The system has an emergency button that is pushed when assistance is needed. Trained staff will communicate with the senior to assess the situation. The appropriate assistance will be notified, which includes an ambulance, family, friends, or neighbors. A variety of vendors are available to provide this service. Prices range and is dependent on the company chosen to provide the service.
