Description:
Comprehensive Assessment and Review for Long Term Care Services is a nursing home pre-admission assessment program which provides comprehensive, on-site assessments of individuals considering admission to a nursing home. CARES staff explore all available options to nursing home placement, recommend and may facilitate alternative placement for individuals who are determined and able to remain in the community.
