Adult Day Transitions (Adult Day Training)
Description
Transitions is a day program for adults who have developmental disabilities. Gulfstream Goodwill operates the Transitions program in West Palm Beach and Ft Pierce. Participants may attend the program one to five days per week. The program focuses on Life Skills, Employment Skills, Personal Growth, and Health and Wellness.

Monday-Friday / 8 am-2:30 pm

Fees
Most programs have no fees. Persons with disabilities wanting to enroll in our adult day training program who are not on the Med Waiver, may be assessed for fees based on a sliding fee scale.

Community Re-entry Program
Description
Provides a support network for ex-offenders that have been released from a Florida DOC facility. The goal is to decrease the chance of returning to a correctional facility and become a productive member of society. Provides employment services, vocational evaluation, assistance in obtaining a Florida Driver's License, Florida ID and Social Security Card. Also provides a peer support group and linkage to community resources.

Released from a Florida DOC facility. Must not be a sexual offender or serious violent offender.

Traveler's Aid
Description
Provides traveler's aid program. Transportation outside of Palm Beach County via bus, train, plane and food, clothing for travel in order to return someone home or to a person who can confirm they will accept the traveler and provide housing to them.

Senior Prevention of Homelessness
Description
Provides case management services to assist senior clients to achieve housing stability; payment of rent, mortgage arrears, utilities and HOA fees. Payment for medical expenses, car repairs, home modifications and other needs that the client may have to prevent homelessness. Roommate matching if appropriate, follow up services 6 months after exit from services to determine housing stability and that the client did not enter the homeless system.

Picture ID, copies of utility bills, lease, mortgage, repair bills/invoices, depending on what services are being requested.
Eligibility
Seniors over age 60 who are at risk of becoming homeless.
