Non-Residential Program

Description

Serves at-risk pregnant and parenting teen and young adult mothers (ages 18-29) and their children who have a safe place to call home but need a helping hand to survive and thrive. Services include case management, mentoring, monitoring health/medical needs of both mother and children, development of life skills, academic/vocational guidance, job search assistance and access to essential resources.

Hours

Monday-Friday / 9:30 am-5 pm (leave message if no answer - phone is checked several times per day)

Eligibility

Must be a single mother, between the ages of 18-29 with family sizes no bigger than 4, must be a Palm Beach County resident.

Residential Program

Description

Provides shelter and mentoring to homeless pregnant and parenting teen and young adult mothers (ages 18-29) and their children in Palm Beach County. Shelter - modest, furnished apartment and assistance with living expenses (independent living model); Mentoring - clients are matched with trained adult female mentors to guide them and monitor the progress and needs of both mother and children; Resources - services include case management; development of life skills; academic/vocational guidance; job search assistance; help in obtaining healthcare, supplies, and job training; and access to essential resources. Scholarship fund assists with post-high school educational/vocational training costs.

DOCUMENTS REQUIRED TO COME IN AND FILL OUT A FULL APPLICATION: DRIVER'S LICENSE, SOCIAL SECURITY CARDS FOR MOTHER AND CHILD, HEALTH INSURANCE FOR MOTHER AND CHILD, BIRTH CERTIFICATES FOR MOTHER AND CHILD, SCHOOL TRANSCRIPT/DIPLOMA/GED CERTIFICATE, COPY OF LAST TWO PAYCHECKS, NAME AND CONTACT INFORMATION FOR 2 PROFESSIONAL REFERENCES AND 1 PERSONAL, AND VERIFICATION OF PREGNANCY IF PREGNANT.

Eligibility

Homeless pregnant or parenting teen and young adult mothers (ages 17-23). Must be mature enough to live independently and be willing to make positive changes in their lifestyles; must continue education and work part-time.

Fees

Minimum of 1/3 total net income to help pay for living expenses
