Description:
Provides individual and group therapy, pscho-educational groups, case management, vocational needs assessment, and supportive services using Client Directed Outcome Informed treatment methodology, psychiatric evaluation, weekly psychiatric services as needed, relapse prevention techniques, medication management, transportation to appointments, medical and community support meetings, linkages to housing and employment services, aftercare services upon discharge. The length of stay is variable with a maximum of LOS at four months.

SACC Referral for Substance Abuse Assessment
Description
Provides DCF mandatory substance abuse assessment referred by the Screening and Assessment Call Center.
Hours
Every day / 24 hours
Eligibility
Must be referred by DCF, CPI or Dependency Case Manager and have FSFN case number.
Fees
No fees charged.
Intake Procedure
Contact Substance Abuse Assessment Call Center by calling 211. Intake will include a conference call with 211. Client and the CPI/or Dependency Case Manager to arrange for appointment.

Description
Provides residential, outpatient and day treatment services for adults who are dealing with substance abuse and mental health issues. It is the aim of Village for Change to provide clients with a safe, secure, supportive environment and an array of professional services, activities and experiences that will assist clients in successfully achieving their goals.

Eligibility
Must be over 18 yrs of age. Negative drug screen, PBC resident, dual diagnosed inclusionary not exclusionary. Primary diagnosis of SA meeting ASMA criteria of free from withdrawal, biomedical conditions are stable, psychiatric condition stable, no risk to self or others, O/P services not sufficient at time of assessment.
Fees
Sliding scale fee; $0 to $100 monthly based on ability.
Intake Procedure
Phone or email to schedule assessment/admission. After assessment is completed and eligibility criteria is met, client will be admitted based on bed availability. Need picture ID, Social Security card, copy of Health insurance care if covered.
