Eligibility
Both parties must be present. Any of the following forms of identification is required: A valid driver's license, a federal or state ID card or passport showing name and date of birth.
Fees

Residents of the State of Florida have the option of attending a premarital course from a registered provider, or waiting 3 days, as Florida law requires, for the marriage license to become effective. If a certificate of completion is provided at the time of application, they will receive a cost reduction and the 3 day waiting period will be waived. There is a fee for the ceremony, payable by cash or credit card. When paying by credit card, a transaction fee of 3.5% is assessed. These fees are in addition to the Marriage License fee.

Adult (age 16 and over) $110 check or money order oly, made payable to US Dept. of State; 3. Child (under age 16) Both parents or proper consent, and child must be present, $80 check or money order only made payable to US Dept. of State; 4. Expedite processing $60 additional fee per applicant, guaranteed 3 week processing; 5. Addition Visa Pages $82.
