Description
Provides comprehensive services; transportation, primary physician and nursing care, medical rehabilitation, lab and diagnostic services, meals, adult day care, home care, specialty medical care, prescriptions, durable medical equipment.
Hours
Monday-Friday / 8:30 am-5 pm
Eligibility
55 or older, reside in service area, have health issues that qualify for nursing home care as determined by Dept. of Elder Affairs CAREs; able to live safely at home.
Fees
No cost to enrollees eligible for Medicaid or Medicare . Fee for others.
