MorseLife Care Management (West Palm Beach)

Description:

MorseLife Care Management provides geriatric care management services to seniors living at home. Care management services provided are: comprehensive assessments; regular in-home visits; phone calls; personal plans of care; assistance with insurance and medical benefits; familiarizes clients with the vast array of services available to them and provides caregiver ( a relative or other person who provides care for a person aged 60 or younger with dementia) and family counseling. Professional consultations are provided.

MorseLife Care Management also provides caregiver counseling under Title III E & EG Palm Beach County.

There are care management fees involved

ELIGIBILITY:

1. Mental Health Counseling: an adult family member; an individual who is an informal provider of in-home and community care for a person aged 'sixty an over'; individuals age sixty who are providing care and support to persons with mental retardation and related development disabilities; a grandparent of older individual who is a relative caregiver who is age sixty and over; grandparent or step-grandparent of a child or a relative of a child by blood or marriage. The caregiver must be diagnosed with a mental health diagnosis, which includes depression and anxiety.

2. For other services: person must be at least 45 years old with a health factor.
