Description
Provides access to life-changing, essential orthodontic treatment for qualified children from low-income families. Children ages 7-18 with a moderate to severe crooked teeth or jaws, good oral hygiene, no more than 4 baby teeth and not already wearing braces.

Required Documents
Tax return or SSI award letter and SCL Dental Referral Form from application packet.
Eligibility
Children ages 7-18 from families with taxable income below 200% of the Federal Poverty level.
Fees
$30 application fee and $650 program fee if approved for treatment through SCL.
Intake Procedure
Complete and submit SCL application available on line or by phone.
