To address the special needs of clients with Alzheimer's disease (AD) and their caregivers through the services of case management, respite in the home and at adult day care programs, memory disorder clinics, caregiver training programs and caregiver support services.

Monday-Friday / 8 am-5 pm

Eligibility
Individuals must be 60 years of age or older and have a diagnosis of Alzheimer's disease or a related disorder, or be suspected of having Alzheimer's disease or a related disorder. Caregivers are also eligible to receive training, respite and related support services to assist them in caring for the ADI client.
Fees
None, donations encouraged. May require co-pay. Co-payments shall be charged to all non-exempt CCE/ADI clients based on the client's ability to pay and include the income of the married, live-in spouse using a sliding scale; currently ranging from $1 for a monthly income range of $1 to $674 to .03% of all monthly income over $4,063.
