The Agency is a non-profit community resource dedicated to serving seniors, older adults, and adults with disabilities. While the aging journey may be a little different for everyone, there are common challenges and issues where the Agency may be able to help. The Agency acts as an advocate for all older individuals, adults with a disability and their caregivers, developing a comprehensive community-based system of care with the aging network to provide needs assessment, and direct client care to help older individuals remain independent. Specific programs include: Helpline Information and Referral services, Intake and screening for in home services, Assistance with Medicaid, SHINE (Serving Health Insurance Needs of Elders), Healthy Living, and volunteer opportunities.

Provides information and referrals on services available to seniors, adults with a disability and their caregivers. Specializing in long term care options counseling. Can identify service providers and agencies to meet their needs. This is a gateway for in home services and assisted living options funded by state and federal dollars. Also offers referrals for private pay resources. Information and referral is available for many needs including: caregiver support, case management, in-home services, insurance counseling, home delivered meals and congregate meal sites, transportation, victim advocacy, assistance with utility payments, and volunteer opportunities.

Description The Elder Helpline are specially trained associates who understand aging and the local agencies, resources and programs which may help seniors age gracefully, support adults with disabilities, and support caregivers. Sometimes, all that is needed is navigation through information and referral. The Helpline associates have access to a database of community agencies with specific expertise. The Elder Helpline may refer a caller to other Agency programs that may provide specialized assistance such as Intake and screening for in home services, Assistance with Medicaid or SHINE (Serving Health Insurance Needs of Elders).

Program: Statewide Medicaid Managed Care Long Term Care (SMMCLTC)
Description
Provides community-based services, Assisted Living and Nursing Home assistance to frail functionally impaired seniors and disabled adults age 18 and above at risk of nursing home placement. The program assists clients to remain in their own homes or Assisted Living facilities. The program also manages clients under Long Term Care Medicaid in a Nursing Home. clients must meet financial and level of care as determined by the Department of children and Families and CARES. Clients choose a LTC Managed Care Plan through an enrollment broker. The plan assigns a case manager to each client who will help develop care plan. Some of the Standard Long Term Care benefits include: adult day care, case management, caregiver training, counseling, Home Delivered Meals, Homemaker, emergency alert and personal care, and Assisted Living.
