Description
Provides outreach program to homeless to assess clients for coordinated entry at Lewis Center. Bringing assessment to them in the streets, parks and other areas frequently visited by the homeless. Providing peer support, connection to community resources and assessment for possible placement into supportive housing programs within the community's homeless services of Continuum Care.
