Eligibility
Must be diagnosed with sever and persistent behavioral health condition such as psychotic disorders, mood disorders and (a) severity of symptoms must be impairing the individuals ability to function in the community therefore placing them at risk for hospitalization or residential treatment care; or(b) has been psychiatrically hospitalized or in residential treatment within the last year and struggling with achieving or maintaining stability of symptoms and functioning in the community.
