Required Documents
Uninsured patients need proof of income in order to be put on sliding scale, photo ID, and insurance card if applicable.
Eligibility
Accept all patients.
Fees
Bills insurance companies for patients covered. If patients are uninsured, patients are put on a sliding fee scale that discounts fees based on patient's documented income.
