Provide in-person group and online diabetic education classes or diabetic self-management classes. Classes are for 5 weeks.

By appointment only.
Required Documents
Driver's license, insurance card, HATT DEC registration form.
Eligibility
Individuals with diabetes or pre-diabetic, or individuals with loved ones with diabetes or pre-diabetes, or individuals who have family history of diabetes and want to prevent it.
Fees
$45.95 for each class or $199 for all 5 classes
