Description
The VA's OEF/OIF/OND Veterans Program provides case management services to Veterans of Operation Enduring Freedom, Operation Iraqi Freedom and Operation New Dawn to ensure that returning combat service members and veterans receive timely treatment and continuity of care for any physical or mental healthcare need they may perceive to have. The program also provides transition coordination from military service to VA services. The transition patient advocate works with each veteran to assure that all needs (housing, healthcare, education, etc.) are met. Case Managers are assigned to Veterans by the Program Manager. Any veteran may request case management. Eligibility: the National Defense Authorization Act of 2008 extends the period of enhanced health care eligibility provided a veteran who served in a theatre of combat operations after November 11, 1998 as follows: Currently enrolled veterans and new enrollees who were discharged from active duty on or after January 28, 2003 are eligible for the enhanced benefits for five years post discharge. Veterans discharged from active duty before January 29, 2003, who apply for enrollment on or after January 28, 2008, are eligible for the enhanced benefit until January 27, 2011.
