Required Documents
Prescription of PPEC Services from the child's Primary Care Physician, Medical Records and other supporting documentation of the child's medical diagnoses and needs.
Eligibility
A child from birth to age 20 that has a medically complex condition that requires a Skilled Nurse and continual care that is prescribd by the child's Primary Care Physician.
Fees
Medicaid, Medicaid HMO's, Children's Medical Services (CMS), Private Pay.
