Copy of birth certificate, copy of all evaluations and exams pertinent to diagnosis, with specialist's signature and date. Copy of Individualied Educational Plan (IEP), progress notes from any previous treatment therapy(ies). Name and numbers of programs and/or therapists that have treated the child.
