The Children's Council of Palm Beach County partners with the Florida Department of Health Palm Beach County and provides funding for three nurse home visitation programs aimed at improving health outcomes for families. These three programs: Healthy Beginnings Nurses, Nurse-Family Partnership and Women's Health Initiative are all part of the Healthy Beginnings System of Care which seeks to improve birth outcomes by providing access to direct health and social services for pregnant women whose chances of having a healthy baby are hampered by such risk factors as poverty, limited access to health care, poor nutrition, age, substance abuse, homelessness, domestic violence, and more.
Description
Services: 1. Registered Nurse curriculum-based home visits with families to improve birth outcomes, enhance child health and developmental outcomes, encourage positive parenting practices, increase intervals between pregnancies, facilitate access to community resources and support economic self-sufficiency and family stability. 2. Nurse of the Day: receives phone calls from the public. The scope of this service is to answer general non-emergency questions regarding basic health issues pertaining to pregnant woman and infants/children from birth to age 3.

Community Health Promotions & Education
Description
Provides technical assistance and consultative services on health promotion and healing education strategies that will impact policy, systems, and environmental changes to improve the health status of individuals, families, and communities. Focus areas include: tobacco prevention and control, chronic disease prevention, repeat teen pregnancy prevention, injury prevention, women's health, minority health issues and safe sleep for infants. Supplemental Nutrition Assistance Program (SNAP) Educational sessions and resources on Nutrition and Obesity. Healthy Weight Initiative: This is a public-private collaboration bringing together state agencies, not for profit organizations, businesses, and entire communities to help Floridaâs children and adults make consistent, informed choices about health eating and Active living. Obesity is expected to contribute to millions of cases of preventable chronic diseases such as type 2 diabetes, heart disease and cancer, costing an estimated $34 billion. To address this important public health issue, the Department of Health launched the Healthiest Weight Florida initiative in January 2013. Diabetes Prevention Program: For All Adult Residents of Palm Beach County. Prediabetic Sessions and Risk Assessment Screening Tool to determine risk. Florida Health Babies Initiatives: Support and Reduce Infant Mortality Community Outreach Events: Schedule and unscheduled outreach presentations and activities on tobacco, Safe Sleep, Diabetes Prevention and Supplementation Nutrition Assistance Program, Clinical Services etc.
