Description
Mental Health Evaluation service is provided upon admission to any service. The purpose of the assessment is to identify the individual's principal reasons for seeking services to clarify needs and preferences and to identify focus of further services.

Monday-Friday / 8 am- 5 pm; After Hours, Saturdays and Sundays by Appointment only.
Required Documents
Proof of Income, Proof of Guardianship, Photo ID, SS Card Proof of Insurance.
Eligibility

Sliding fee scale, Medicaid, Medicaid HMO's, Medicaid Waiver for Aged and Disabled Adults, Healthy Kids, may qualify for services provided by a children's collaborative agreement. Funding available for uninsured, underinsured and undocumented individuals.

Psychiatric services are provided in a clinical setting, they include: Psychiatric Evaluations, Medication Management, taking of Vital signs. Medication Management services are provided to ensure persons served get maximum benefits of appropriate pharmacotherapy practices.
