Homeless Services Rapid Rehousing Program

Description

Serves individuals who are homeless through a continuum of care, outreach, emergency shelter, intake and assessment, and support services, with the goal of gaining or regaining economic self-sufficiency. Services are provided through regularly weekly outreach and Case management at The Lewis Center.

Eligibility

No barriers however, for intake to the Rapid Rehousing Program client must meet the following definition of homeless: Individual who lacks a fixed, regular and adequate nighttime residence, and/or individual in an emergency shelter, or staying in a public or private place not designed for regular sleeping accommodations for human beings. Homeless individuals are served based on HUD definition. 1. On the streets or in a place not meant for human habitation. 2. In an emergency shelter, transitional housing program, or travel trailer park. 3. In housing, but being evicted within 14 days and not having resources or support networks to obtain housing. 4. Fleeing domestic violence. 5. An individual/family doubled up with others due to loss of housing. All referrals are based on prioritization of needs, and not on a first come first served basis.

No fees charged.

Intake Procedure

Clients need to call the Lewis Center (561) 904-7900 for assessment. Clients are assessed at the Lewis Center 7 days a week.
