Description

HCRV's goals are to prevent homelessness, reduce the impact of medical, psychiatric, and substance abuse problems upon community re-adjustment, and decrease the likelihood of re-incarceration for those leaving prison. HCRV services include: outreach and pre-release assessments services for Veterans in prison; referrals and linkages to medical, psychiatric, and social services, including employment services upon release; Short term case management assistance upon release.

Eligibility

Must be Veteran. Honorable or general discharge; Two years in a row of active duty. No length of service rule for veterans from Iraq or Afghanistan. No length of service rule for veterans enlisted before September 9, 1980; DD214 Form
